Job description

2nd Shift position: Monday - Friday, 4:00pm-12:30am

Under general supervision and working as part of the Crisis Stabilization Unit, performs intake referrals, admissions, diagnostic assessments and discharge planning to individuals who present in need of mental health services. Provides crisis intervention services to adults who present in a mental health crisis. Makes treatment recommendations, and develops, completes and monitors individualized service plans (ISPs). Completes and maintains service documentation according to agency and regulatory standards.- Conducts Crisis Assessments, Crisis Plans and discharge planning that are sensitive and responsive to client cultural and ethnic backgrounds, functioning and needs, as well as environmental and situational factors by collecting and evaluating client information.
- Admits clients to the Crisis Stabilization Unit.
- Formulates diagnoses according to DSM-IV criteria by gathering, organizing, documenting and analysing all input sources. Communicates results of assessments and makes recommendations.
- Conducts crisis intervention services with clients at the CSU when needed.
- Assists clients to become actively involved in their own recovery.
- Conducts on-to-one meetings with clients to discuss both current and anticipated issues, to teach coping strategies and assertiveness and to assist the client with practicing strategies in order to solve or cope with presenting issues.
- Conducts group counseling services in order to assist in the client with psychiatric stabilization and to improve the client's re-entry into the community.
- Coordinates discharge planning from the time of admission, including linkage and referral to formal and informal support services in the community
- Ensures that services are performed and documented in accordance with established policies, procedures and other pertinent protocols of the agency, the Ohio Department of Mental Health administrative rules and any other pertinent regulations.
- Develops and monitors individualized service plans (ISPs), including coordination of interdisciplinary team meeting for communication and planning. Establishes treatment goals and determines treatment methodologies.
- Monitors the progress toward treatment goals and effectiveness of interventions by evaluating and adjusting services.
- Promotes long-term crisis management and stabilization by working with families and/or significant others.
- Assists clients and family members in decision-making by providing information about service choices.
- Promotes prompt delivery of ongoing services by collaborating with healthcare providers in service planning.


Must have a LSW or LISW, PC or PCC. Bachelor's degree in social work, psychology, sociology or related field, with a minimum of one year professionally supervised work in the delivery of mental health, chemical dependency and/or social services. Must have a valid drivers license with access to a vehicle
